The law of fraud typically requires that the one accused of fraud be proven to have acted intentionally or with wanton disregard for the results of the action. In cases of Medicaid fraud the typical case involves misstatements by people in Medicaid applications for insurance. Sometimes, this is due to simply a lack of care in filling out forms properly.
